Notice for PG (2015-16) admission - Sir John Wilson School

Dear Parents,

This is to inform you that Admission Forms for Playgroup (2015-2016) will be available from the school office (Road-77, House-5, Gulshan 2) on the following dates:

Distribution of Forms
2^nd November 2014 (From 8:30 am to 2:00 pm)	For siblings and staff only
3^rd, 5^th and 6^th November 2014 (From 8:30 am to 2:00 pm)	For others
Submission of Forms
Date: 9^th November 2014 Time: 8:30 am to 1:00 pm	Siblings and staff
Date: 15^th November 2014 (Saturday) Time: 8:30 am to 1:00 pm	For others

If you are interested in admitting your child/children in Playgroup for the Academic Year 2015 – 2016, please buy the admission forms within the times mentioned above.

Please bring a photocopy of your child’s birth certificate or photocopy of child’s passport and two passport size photos of your child on the day of submission.

Please note:

1. Form Numbers will be given only at the time of submission of the forms. All forms are collected from parents on a first come first served basis.

2. The age range for Playgroup 2015-2016: Born between 1^st August’ 2011 to 31^st July’2012.

3. Form charge (tk -600) Non-refundable.

4. Total seat capacity for Playgroup 2015-16 is 140. The first 240 submitted forms will be called for an Assessment Interview and rest of the forms and names will be placed on a waiting list.

5. For any sudden or officially declared closure, the form distribution and submission will take place on the next working day.
